rate the behavior #1 – 5. For example, 1 = Rare 0-2 incidents, 2 = 3-4 incidents, 3 = Moderate 5-6 incidents, 4 = 7-8 incidents, and 5 = Frequent 9+
incidents. The scale you develop will vary per student and behavior. When collecting data, write the date at the top of the form, and circle the
