PARENT QUESTIONNAIRE FOR THE

COLLEGE COUNSELING PROCESS

During the second semester, counselors invite all juniors to meet with them to begin the college

exploration process. As a parent (guardian), you can help with this process by completing this

questionnaire. Parents usually know their child best. This information is not only valuable in

discussing post-secondary possibilities with you student, but also in preparing the counselor

recommendation which many colleges and universities require. Specific detail adds substance to

our recommendations. We are not seeking detailed answers to these questions, but your essential

thoughts. We will also ask your son or daughter to complete a questionnaire before we write the

counselor recommendation.
