INSTRUCTIONS
1.
The California National Guard Board of Corrections to Military Records can only address service records while the applicant was a
member of the California Army National Guard.
2.
The California National Guard Board of Corrections to Military Records cannot consider any type of discharge resulting from a
sentence given by a general court martial.
3.
Time limit for filing an application. Application for Correction to Military Records while a member of the California Army National
Guard must be filed within three years of the discovery of the alleged error or injustice. Failure to file within the three year time limit
prescribed may only be approved by the board when the board finds it to be in the best interest of justice to do so.
COMPLETING THE CA ARNG FORM 149.
1.
All information must be typed or printed (Items on the form are self-explanatory unless otherwise noted below).
2.
If space is insufficient on the front of the form use the "Remarks" block below for additional information or attach an additional sheet.
3.
Block 1a. Enter the name in which you served under while a member of the California Army National Guard.
4.
Block 1d. Enter the address to be used for all future correspondence regarding your application. If your mailing address changes
while your application is pending, you must notify the CA NG Board of Corrections to Military Records.
5.
List all attachments and enclosures in Block 9.
