REQUEST FOR ACCESS TO INFORMATION
Subject to certain exemptions you have the right to be told whether any information is held
about you and a right to obtain a copy of that information.
It is a criminal offence for you to be required to make a subject access request to any police
force for details of any or all of your convictions/cautions or confirmation that you do not have
convictions or cautions in connection with:
your recruitment as an employee;
your continued employment as an employee; or
any contract with you for the provision of services by you, for example as a self employed
person.
It is also an offence for you to be required to make such a subject access request as a condition
of being provided with goods, facilities or services, e.g. by an insurance provider or a housing
provider.

The correct route for obtaining details in such circumstances is through Disclosure Scotland
who can advise what can be supplied and in what circumstances.
If you only need a copy of your unspent conviction history, this can be obtained by applying to
Disclosure Scotland,
(Tel: 0870 609 6006) who will provide
you with a Criminal Conviction Certificate (a Basic Disclosure).
This Certificate can also be obtained online.
If you require a criminal record check for the purpose of emigrating to Australia, Belgium,
Canada, Cayman Islands, New Zealand, South Africa or the United States of America please
contact the ACPO Criminal Record Office at

If you require a copy of information held by Police Service of Scotland (PSoS) (including historic
information held by one of the eight previous Scottish Police Forces), please complete all the
sections below and return this form, together with the £10 fee and identification, to any
operational Police Office in Scotland.
