term.

_____ You must submit copies of transcripts from previous institutions to verify transfer credits.

_____ All courses you register for must be required for your program of study or elective requirements. You will be responsible for any

tuition and fees for any course not applicable toward your degree program. If you are not sure whether a course will meet a

degree or elective requirement, please ask your academic advisor before registering for the course.

_____ You may retake a course if you failed the course with a grade of F. VA benefits will not cover a retake of a course with a grade

of D or higher unless your degree requires a higher grade. The UD Veterans Services Office needs confirmation that you

earned the F from your instructor.

_____ You are required to notify this office and complete

VA Form 22-1995

if you change your program of study. Your certification

status will be considered inaccurate until completed.

_____ You are required to notify this office any time you add or drop courses. Failure to do so will result in incorrect payment and/or a

debt to the VA.

U - Unsatisfactory; no credit awarded

NC - No credit awarded
