Note:

1) If this request is made jointly, then both parties' complete information and signatures must be

included.

2) If you have not obtained credit while living at your current address, or if you have been the victim

of a severely mixed file or identity theft, then a credit reporting agency may legally request

additional proof of your identity showing your current address, such as a photocopy of your

driver's license, an insurance or bank statement, or a utility bill. In these cases, it will save time to

include that additional information with this initial request form.

DO NOT attempt to obtain someone else's credit report; to do so is a serious crime.
