List of Required Documentation and Information for N-400
**THE FOLLOWING DOCUMENTATION/INFORMATION MUST BE PROVIDED TO COMPLETE
THE N-400 APPLICATION. IF ANY OF THIS INFORMATION IS MISSING, WE WILL NOT BE
ABLE TO COMPLETE YOUR APPLICATION**
Documentation you MUST bring:

Your permanent resident card
2.
Social Security Card
Driver’s License or State Issued ID Card
3.

2 Passport Photos (2 in. x 2 in.) - you must be facing forward in the photographs (DMRS can take these
photos for a cost of $8)
5.
Current Marriage Certificate
6.
Certified Copies of all divorce decrees and annulments
7.
Selective Service Registration (men only)
8.
Certified copies of documents related to arrests/convictions
9.
$7.00 cash for postage to mail the application
10.
Money Orders Required:
A. $595.00 - Money Order payable to Department of Homeland Security for the N-400
Application Fee.
B. $85.00 - Money Order payable to Department of Homeland Security for the Fingerprint Fees.
11. Fee Waivers :
To apply for a fee waiver, please provide documentation of means-tested benefits received, such as
Medicaid, Food Stamps, Temporary Assistance to Needy Families (TANF), or Supplemental Security
Income (SSI),
Information you MUST provide – please complete on a separate sheet of paper

Information about your residence with the exact date where you have lived during the last five (5)
years.
A. Current home address – from (month, day, and year) to present include street number and name,
apartment number, city, county, state and zip code.
B. Mailing address- if different from current home address, name street number or PO Box#, city,
state and zip code.
C. Previous addresses past five (5) years-include dates of residence (from month, day, year to
month, day, year) include street number and name, apartment number, city, county, state and zip
code.
