Information about ALL of your children, including children who are alive, missing, deceased, born in
other countries or in the U.S.; under 18 years of age or adults; are married or unmarried; living with

If you have been convicted of a crime or been arrested, you must obtain:
A. An original, certified copy of the Official Court Judgment and Sentence for all arrests (even if
you were not charged or convicted), convictions (even if your record was cleared or expunged)
and crimes you have committed for which you were not arrested or convicted. The Court
Judgment must indicate that it covers both felony and misdemeanor arrests (in El Paso County
you must go to both the County and the District Clerk’s offices). If the court has not record of the
case, you must provide a certified document that states no information is available on the case.
This certified document must reflect that both misdemeanor and felony records checks were
conducted. If the Court Judgment indicates any convictions (including guilty or “no contest”
pleas), you must also include proof that all penalties imposed (probation, jail sentence, fines,
court costs, community service, etc.) have been completed.
B. The Police Incident Report for all your arrests will also be required. Even if you have
committed a minor crime, CIS may deny your application if you do not tell CIS Office about the
incident
C. Clearance letters from Police and Sheriff’s Departments are also required even if you have
never been arrested
