1Mail Forwarding Service Agreement
This Mail Forwarding Service Agreement is entered into between Mail Box Center Limited, 761 W. Sproul Road,
Springfield, Pennsylvania 19064 (hereinafter referred to as “Center”) and the person identified below as Customer.
Center and Customer have entered into a Mailbox Service Agreement (hereinafter referred to as the “MSA”) and
Customer has provided a USPS Form 1583 to Center. This Mail Forwarding Agreement is considered an
Addendum to the MSA and all of the terms and conditions of the MSA are hereby incorporated herein as if they
were specifically set forth herein. Customer agrees that if the MSA is cancelled for any reason, Center may suspend
or cancel the mail forwarding service described herein.
Customer hereby requests that Center forward all mail and packages received in Customer’s mailbox to Customer
upon the terms and conditions set forth herein. Center assumes no liability for damages, direct or indirect, known or
unknown, including consequential damages, as a result of the use of this service.
Customer agrees to comply with all United States Postal regulations. Any penalties or fines resulting from any
violation of this condition will be the sole responsibility of Customer, and Customer agrees to indemnify and hold
Center harmless for any damages, penalties or fines, including attorneys’ fees, incurred as a result of any such
violation.
Customer agrees to prepay and maintain a mail forwarding deposit account balance with Center of at least fifty
($50.--) dollars. All shipping and packing charges of Center to forward mail and packages to Customer will be
charged by Center to this account. If said account reaches a balance of four ($4.--) dollars or less, Customer agrees
that Center may suspend the mail forwarding service until the account is brought to the minimum balance.
Until written notice is received by Center, Customer’s mail and packages are to be forwarded to the mail forwarding
address set forth below:
