You can receive Veterans Educational benefits for all courses taken at a supplemental school once your

ASU (parent school) advisor has approved them. The classes must apply to your degree program.

Please take this form to your ASU advisor and have it completed and signed. Then return it to Veteran

Benefits & Certifications at ASU as soon as possible. Veteran Benefits & Certifications will sign it in the

ASU Certifying Official area, make a copy for your file, and send a copy to the supplemental school. You

are responsible for informing the supplemental school of your intent.

They will complete an enrollment certification (22-1999) and submit the enrollment information to the

Regional Processing Office in Muskogee, OK.

If you are a Vocational Rehabilitation (Chapter 31) recipient, your case manager will need to send an

additional 1905 directly to the supplemental school Veterans Services Office. The supplemental school

will certify your enrollment there and submit a 22-1999 to the Department of Veterans Affairs office.
