Summer Assignments

All students entering 5

grade are required to read the following book:

Shiloh by Phyllis Reynolds Naylor

Attached you will find a book report form for Shiloh. The report is to be written in

script. You must design a cover using construction paper. The title and author must appear at

the top of your cover along with your name in the lower right hand corner. A picture related to

the book should also be on the cover. It will be collected the first day of school. You will also

be tested on this book when you return in September, so it is required that you keep a brief

chapter by chapter summary of the book. This book can be purchased at any major bookstore or

online. The grades you receive on this assignment will count towards your first reading grades

for the school year.

All 5

graders are to have the multiplication tables committed to memory.

Attached is a copy of the tables. You will have a timed multiplication test when you return.

All students are expected to write in script from the beginning of the school year.

Attached is a cursive writing chart.

Also included in this packet is a 5

grade supply list as well as a Student Information

sheet. Please fill out the information form and return it the first day of school. It is important

that you start off the year on the right foot, so make sure all assignments are completed.
