DACA RENEWAL
ELIGIBILITY SCREENING FORM
To determine whether there are any red flags in your case or to request ILCM’s
assistance, please answer the questions on this confidential questionnaire. Whether or
not you identify any red flags, we strongly recommend that you consult with ILCM or
another experienced immigration attorney before filing any application with U.S.
Citizenship and Immigration Services (USCIS). *Please note that this questionnaire is
only for people who have already received DACA and now want to renew DACA.
1. When does your DACA expire? __________________ (The expiration date on your employment
authorization card is the same date that your DACA expires).
2. Is your expiration date more than 150 days (5 months) away?
If you answered “yes” it is too early to file your application. USCIS may reject

applications that are filed earlier than 150 days before the expiration date. However,
it may not be too early to consult with ILCM or another immigration attorney. ILCM
will consider requests for assistance from people whose expiration date is less than 6
months away.
If you answered “no,” it is important to file your application as soon as possible, ideally

no later than 120 days before your expiration date.
3. Have you been arrested, charged or convicted for a misdemeanor or felony offense since
your initial DACA was approved? This includes tickets, juvenile arrests, and even cases that
were dismissed or expunged.
If you answered “yes” you must get your arrest or conviction records and consult with

ILCM or another experienced immigration attorney before filing any DACA renewal
application, to make sure you are still eligible.
If you answered “no,” proceed to the next question.
