To receive the Costco Cash Card rebate, the following requirements must be met:

1. Purchase and activate a qualified AT&T device on a 2-Year New Agreement or a 2-Year Qualified Upgrade

at Costco or on

 iPhone 5s 16GB: $75 Costco Cash Card

 iPhone 6 16GB, 64GB or 128GB: $80 Costco Cash Card

2. Purchase Order and Activation must be made between the dates of 11/21/2014 – 11/30/2014.

3. AT&T service must be active at time of redemption.

4. Complete this certificate and mail along with a photocopy of your Costco receipt and AT&T agreement or
