Steps to request transcripts for colleges:

1) Complete the transcript request form; completion of all mailing addresses and deadline dates is required!

2) Print and complete Secondary School Reports (found on college/university websites) for all colleges listed on the transcript request form

3) Include $2 per college listed below (this fee is not required for Common Application Schools)

4) Turn all Secondary School Report forms in with this transcript request form and your money. Paperwork will only be accepted if complete!

DEADLINES: All paperwork must be turned in to your counselor at least 30 days before the college application deadline

Disciplinary Reporting Policy:

Many colleges request information from the high school about disciplinary violations and/or academic misconduct. Suspensions that have occurred while at AHS will be reported to a

requesting college or university. Applicants are expected to notify colleges and universities in the event of any change to their application, including disciplinary infractions that occur

after an application is submitted. Likewise, the School Counselor is required to report any significant changes in the applicant’s status between the time of application and graduation,

whether the change is related to academics or discipline issues.

To protect the confidentiality of students, we ask that you sign the following statement permitting us to release cumulative record information:
