National Honor Society Application
Congratulations on being considered for membership to the Pascack Hills Chapter of the National Honor Society.
While it is your high grade-point average that has qualified you for candidacy, your demonstration of service,
leadership, and character are also a necessary step toward achieving admission into the NHS. Thus, you must
complete and submit all parts of this application by Tuesday, March 6 at 3:00 pm.
I have completed all written parts of this application in full.
I have stapled all parts of the application in the following order:
Completed forms, Parts I-V
Service documentation sheets, completed and in sealed, signed envelope(s)
Completed and signed Extra/Co-Curricular Activities – Substance Use and Tobacco Policy Contract
Completed tutoring information form
I have proofread all written responses and checked for appropriate signatures.
I am ready to submit this to Mrs. White in the library by Tuesday, March 6 at 3:00 pm.
Feb. 13 – distribute forms
March 6 – forms due to Mrs. White
March 12 – 16 – consideration by faculty council
March 19, 20 – determination letters go out
April 16 – brief meeting (mandatory)
April 17 – mandatory induction ceremony
