Directions: Neatly, in blue or black ink, maintain a record of reading your book outside of English class at least three times per week
for a total of 120 minutes or more. Be sure to fill in dates, write out the book title for each entry, and record time spent reading.

You are required to read a minimum of one Accelerated Reader (AR) book every four weeks. The availability of AR books within
the POLYTECH Learning Resource Center can be explored on the online library catalog at
Additionally, students have access to 160,000 AR quizzes on Accelerated Reader Enterprise but may need to check a book out of
a public library if necessary. Students may investigate available titles at All books for the project must
be within your assessed reading range (left corner below) in order to be of educational value and approved for the assignment. If
you should finish the book and complete the computerized AR quiz prior to the end of the quarter, you are required to begin
another book in order to complete weekly reading log requirements and to have a book to bring to class for Reading Prep every
Friday. Please note that no credit will be given for AR quizzes that do not meet the reading level criteria. Students should log in a
minimum of three reading sessions (a total of 120 minutes or more) per week as an ongoing reading assignment for homework per
the directions at the top of the page. I will use the computer quiz score for your book as well as the completion of this log for grades
due on _______________________. If you should complete more than one approved AR book within the four weeks, you may
choose the best of the quiz scores to use as your grade for that quarter. Parents should approve the content for each book on the
reading log before you read and should sign below indicating approval of content.
