• leaving for summer and expect to receive mail

Campus Mailbox Forwarding Policies
• A change of address form must be submitted prior to the end of the semester and moving off campus
• Change of address forms may not be submitted by telephone or fax
• The form must be completed and signed by the same individual who is changing their address
• Mail received after the change of address form deadline is returned to sender
