General Waiver and Release Form for

WELCOME WAREHOUSE Recipients

I/We, ___________________________________________________ (your name/s), on behalf of myself

and any family member, do participate as a RECIPIENT (the Recipient) in Welcome Warehouse, Inc. (the Program)

solely as a Recipient of pro-bono volunteer assistance.

I/We are participating solely to receive free help that I/We are soliciting and receiving from volunteers

participating in the Program.

I/We wish to participate as RECIPIENT of Welcome Warehouse, Inc., assistance and I/We sign this

Waiver and agree to each of the following:

1. I/We release and hold harmless any and all volunteers performing assistance from any and all claims that

may arise out of any program, activity, or service through Welcome Warehouse, Inc.

2. I/We release, waive, discharge, and covenant not to make a claim against or sue (a) Welcome

Warehouse, Inc. or any of its employees, agents, director, officers, volunteers, and (b) any contractors

related to Welcome Warehouse, Inc. or their employers, suppliers, vendors, from all liability for any

claims relating to their participation in any Welcome Warehouse program, activity, or service, including,

but not limited to, recommendations, advice, inspections, or repairs which they may or may not have

given or provided to us.

3. I/We agree to indemnify, save and hold harmless (a) Welcome Warehouse, Inc., or any of its employees,

agents, officers, volunteers, and (b) any contractors or their employers, suppliers, vendors from any and

all loss, liability damage or cost that may be incurred due to any recommendations, advice, inspections,

repairs, or other work or volunteer activities which the volunteers from the Program provide to us or our

organization.

4. I attest that no oral representations, statements, or inducements apart from the foregoing written

agreement have been made.

5. I agree to read and voluntarily sign this Release, Waiver and Indemnity Agreement.
