Please write the child’s full name, dental ofﬁ ce and age on the back of this sheet. Either the parent or the dental ofﬁ ce staff may submit the

coloring sheet to Delta Dental of Kansas, Attn: P.A.N.D.A. Committee, P.O. Box 789769, Wichita, KS 67278-9769. All coloring sheets must be

received no later than April 30, 2012. Entries will be voted on by Delta Dental of Kansas employees. Winners for both categories (ages 0-3,

ages 4-7 and ages 8-10) must be available for a quick photo with the dental ofﬁ ce staff between May 14-21. The photo will be published in

Delta Dental’s dentist newsletter and sent to the local newspaper.
