Our simple application process...

1. Completing the Application Form

To make the joining process as painless as possible we will complete the application form for you as part of the

interview process, just remember to have the details we need to hand.

If you prefer of course, feel free to complete and return the application form yourself sending it to

along with your copies of your important documents (below).

2. Send us your Documents

You will need to provide us with copies of the following:



Your up-to-date CV (in a Microsoft Word format)



GMC Registration



Your Qualifications including PMETB / CCT



Performer’s List Details



Existing CRB Disclosure (if applicable)



x2 References (covering the last three years)



Occupational Health Questionnaire and Immunisation Certificates



Your Passport and Driving Licence



A recent Utility Bill dated within the last 2 months or a Council Tax bill dated within the last 12 months



x2 Passport Photographs



Proof of Medical Indemnity Insurance



Proof of Eligibility to work in the UK (if applicable)



Relevant English speaking qualifications if English is not your first language (if applicable)

3. We will arrange an interview

We will arrange an informal interview, this is usually done online via Skype and using a webcam (we will send you a

webcam & instructions once we have arranged an interview date – it is really quick & easy too).

4. We will request CRB Clearance

As part of our registration process, the cost of your CRB must be paid in advance. The current cost of an enhanced

the CRB is £50.00, this is refundable in full once you have completed 50 hours of shifts with Hallam GPs.

As soon as we have received your documents, we will send you online access to complete the enhanced CRB

disclosure form.

We must have

a copy of your Passport, Driving Licence, a Utility bill dated within the last 2 months

(or Council Tax bill dated within the last 12 months) and your photograph.

On average it will take approximately 2-4 weeks for your CRB clearance to come through.

Please note:

If you have had a CRB check carried out by your current employer within the last 12 months we can use

this in the interim and you can start working immediately.

4. We will request Occupational Health Clearance

You will need to complete and return our Occupational Health Questionnaire along with your immunisation certificates.

We will request and pay for your Occupational Health Clearance, we also provide

annual renewals free of charge

too!
