CHECKLIST
Submit the following documents with your application. Check
each box after enclosing the
item. This document checklist is individual.
Failure to provide a complete application or the necessary supporting documents will result in
return of your application without being processed. If you are sending more than one application
and one of the applications is incomplete, all of them will be returned.
Visa form delivery receipt (RECIBO DE ENTREGA DE REQUERIMENTO – RER)
The RER is generated after you fill out an online form available through
https://scedv.serpro.gov.br/
It must be signed (in the proper box) by the applicant ≥18 years old or the adult(s) responsible for a
minor. The RER should come on top of all the supporting documentation (stapled).
Passport or “laissez-passer”
Valid for at least 6 (six) months, in good condition.
If you are 18 years old or older, the passport must be signed in the appropriate space.
Photo (measuring EXACTLY 2x2 inches)
One (1) of the front face professionally taken against a white or off-white background.
You must glue it to the RECIBO DE ENTREGA DE REQUERIMENTO. The box between the bar codes is
just a placement guide. DO NOT cut or fold the photo as it needs to be scanned.

Authorization form (only for minors under 18 years-old to apply for a visa)
Downloaded from our website, completed and signed, along with:
Proof of parenthood (long form of birth certificate showing both parents names).
Parent’s death certificate (if applicable).
Judicial document as proof of sole custody/guardianship (if applicable).
Fees (in Canadian dollars paid by CERTIFIED CHECK or MONEY ORDER only)
We do not accept as form of payment: cash, credit card or personal checks.
Certified checks or money orders must be made payable to the Consulate General of Brazil.
You must write the name of the applicant(s) in the reference line.
The $25 handling fee does not apply if you are dropping off your application in person.
Do not staple the certified check or money order to the application.
Prepaid return envelope obtained from Canada Post (for all applications sent by mail)
The only two types of return envelopes that we accept are Priority™ or Xpresspost™ (both from
Canada Post) and they must be preaddressed. We do not accept return envelopes from other
courier companies such as FEDEX, DHL, UPS, to name a few, or envelopes with stamps.
