To receive financial aid, federal law requires the student to be either a United States citizen or an eligible non-citizen. So that we can verify
your status, please complete the certification statement below and attach clear photocopies of the Indicated documentation. Please note that your
documentation will be submitted to the Immigration and Naturalization Service for confirmation. Please
check the appropriate box below:

Citizen born in the United States-
Attach a copy of your U. S. birth certificate, Baptismal certificate or U. S. Passport.
Citizen not born in United States-
Bring in form FS-240 bearing the embossed seal including the words “United States of America” and “
State Department”(we will make a copy); or
Your Certificate of Citizenship (we will make a copy) ; or
Your Certificate of Naturalization (we will make a copy) ; or
A copy of your U. S. Passport
Non-citizen Nationals (natives of American Samoa or Swain’s Island) –
Permanent residents of the Northern Mariana Islands or the Trust Territory
of Pacific Islands –
Attach a copy of your U. S. Birth certificate or U. S. passport.
U.S. Permanent Resident – Your name on Immigration documents:

Attach copies of both sides of your Alien Registration Receipt card (I-551, I-151 or _-551C); or
A copy of your passport, valid through first day of classes, stamped “Processed for I-551..” showing
copy of the picture Identification page); or
Copy of your Departure Record (Form I-94) valid through the first day of classes, stamped “processed
for I-151…..” showing the expiration date, or stamped “Temporary Firm I-551” showing the expiration
date. Include copy of the picture identification page.
