REQUEST fee. It is the coach’s responsibility to check players’/team’s availability ahead of time and make your request accordingly. Weber County is not responsible
to make-up forfeited games. Thank you for your cooperation and understanding as we try to accommodate as many requests as possible.
I agree to indemnify and hold Weber County, it’s officers, officials, employees and agents harmless from any and all claims and to defend
said county and pay for all costs incurred in the defense of claims against the county, it’s officers, officials, and employees, and agents,
which arise out of the activities of the applicant or any person participating as a member of the applicant’s group.
All team members must sign individually. If not 16 years of age, a parent or guardian must sign. By not signing, the player(s) are
not eligible to play ball and will be considered illegal player(s).*No player shall play for more than one team in the same league.

Refund Policy: Refunds requested 15 calendar days or more before league play will result in a $100 (deposit) penalty.
Refunds requested within 14 calendar days of play will result in a 50% penalty.
Refunds requested after league play starts will result in a 100% penalty.
Full refunds will not be granted unless league is canceled by the Recreation department.
Coaches are responsible for all league fees, rosters and release forms.
In the event that Weber County Recreation is not able to accommodate this team as part of the requested league, Weber County
Recreation may make accommodations for the team with a neighboring recreation organization or will refund payment. Team coaches
will be notified if this becomes necessary.
