Child Care Subsidy – Camp/Recreation Programs

Agreement between Parent/Guardian and Licensed Child Care Program/Home Child Caregiver

This form is for parents who:

Are currently in receipt of Child Care Subsidy,

Have school-age children (age 6-12 years) that are currently attending a licensed child care

centre or home child care program,

Are requesting to use a camp program during March or Summer Break.

Registration for camp programs starts in March/April. It is important to make sure the camp program

has space for your child before completing this form. One form must be completed for each camp

program. This form must be received by the Child Care Subsidy office a minimum of 2 weeks before

the first day of the camp program.
