Child Care Contract

This contract is entered into by and between___________________________(“Provider”)

and________________________(“Parents”) for the provision of child care

Children of new clients will be placed in my family child care home on a two week trial basis. This allows all parties

to get acquainted, and if there are any personality conflicts, disciplinary problems, etc. each party has the

opportunity, at an early stage, to terminate child care in my home and seek alternative care.

Registration

The following forms must be completed and received by the Provider before care begins:

1. Admission Record

2. Health Status Form

3. Emergency, Medical Authorization Form (Notarized)

4. Wildwood Food Program form

5. Signed Contract

The information on these forms must be kept current. If there is any change, the Parents do hereby agree that they

shall notify the Provider immediately. The Health Status Form must be updated and signed by your child’s physician

on a yearly basis.

B. It is imperative to the Provider, the Parents and the child that consistent hours of business are established so

that each party knows what to expect. It is a hardship for the Provider if they are expected to visit with the

Parents while trying to give quality care to the other children. I ask for your cooperation in limiting the

length of your stay as you drop your child off and as you pick them up from my home. However, please

feel free to inquire about your child’s day or ask any questions you might have pertaining to your child’s

care.
