INDIVIDUAL PERSONAL GUARANTEE

For and in consideration of J. B. Parks Wholesale Florist, Inc. extending credit at the undersigned’s

request to _______________________________________________________________ (hereinafter referred

to as “company”), the undersigned (whether one or more) jointly, severally and unconditionally guarantees

the full and punctual payment when due of all indebtedness now and hereafter owing by said company,

and personally guarantees to J. B. Parks Wholesale Florist, Inc., the payment at 3510 Ross Avenue, Dallas,

TX 75204, of any obligation of the company whenever the company signed to pay attorney fees and /or

court costs assessed by a court, or paid by J. B. Parks Wholesale Florist, Inc., should the account balance

be placed with an attorney or agency for collection. I/We further agree to resolve all disputes for payment

in Dallas County, Texas.

The undersigned waive notice of J. B. Parks Wholesale Florist, Inc. acceptance hereof, of the accrual,

renewal and extension of the indebtedness, of the company’s default, and of the accrual of the

undersigned’s liability hereunder, as well as grace, notice, presentment for payment, and protest with

respect to every portion of indebtedness.

I/We further authorize J. B. Parks Wholesale Florist, Inc., to investigate the undersigned’s personal credit,

including requesting a credit report from any credit-reporting agency.

I/We further authorize J. B. Parks Wholesale Florist, Inc., or any of our affiliates to notify me as a

customer in any method, including but not limited to facsimile, email, or mail, information regarding our

inventories, prices, or other pertinent information regarding our business.
