WAIVER OF LIABILITY AND ACCEPTANCE OF SOUTH EAST QUEENSLAND FOOTBALL POLICIES

I, the undersigned, hereby bind ourselves, our heirs, assigns and personal representative to waive South East Queensland Football and any and all of their

directors, agents, officers, coaches, committees, representatives, members and/or member's parents, as well as any other football associations, teams, schools or

officials, or team members, against or with whom the participant named below may be participating or practicing, from any and all claims or rights to damages

for injuries or losses suffered directly or indirectly in training, workouts, attendance, participating in or travel from practices or competition. In case of injury or

illness of the named party, I hereby give consent for any paramedic, licensed physician, or other persons authorized by SEQ Football or any referee or team

captain, to transport the named participate and to obtain and/or render necessary First aid and/or medical treatment. I hereby give my consent for the above

named to compete in the football league, hereinafter referred as South East Queensland Football league. Further, I hereby waive all claims for accidents or

liability of any kind against the South East Queensland Football league, program, competition, its coaches, managers, directors, advisors or members of this

organisation to include, but not limited to South East Queensland Football and any of its owners/lessors/lessees of any site where a member and/or team, practice

or play. I acknowledge that soccer/football is a body contact sport from which injury, disablement or even death may occur. I also acknowledge that I am

physically fit to participate in the football program. I agree to abide by the rules of this organisation. I understand this means that I cannot sue South East

Queensland Football or any person associated with South East Queensland Football in the event of injury and that I must rely on our own health insurance or

other financial resources in case of injury. I understand that I am responsible for the equipment required to be able to participate in this league, such as but not

limited to shin pads, team strip and football boots.
