MEAL SIGN-UP SHEET FOR NON-PRODUCTION KITCHEN SITES
Per your site’s plan: please sign below to insure enough food is prepared, and pay the designated receiver of
payments at your site. Contact your center management for information.
Please find your site’s prices below, as each school district or food service has its own pricing. (NOTE:
Price may be changed per the school district.)
Classroom staff will only be charged for full lunches or milk (not breakfast, snack, or Family Night meals).
Non-classroom staff will be charged for all meals and milk (not Family Night meals).
Checks must be made out to the ESD 113.
Payments must be in a secure location and forwarded monthly, with this form, to the Fiscal Coordinator.
