Purpose:
Use this form to record and/or change a commercial driver's self-certification category for the driver's type of commercial
motor vehicle operation as required by VA Code §46.2-341.12(G).
Instructions:
Review the "To Submit Your Self-Certification" section below to determine how you may submit this information.
Complete the Self-Certification section and attach any required documents.
To submit ON-LINE, visit and under "Submitting Your Documentation" click "Upload
Online".
You may also submit your information by MAIL to the above address, attention CDL/NDR Work Center or FAX to
804-367-6692 or by visiting any DMV customer service center.
DMV will process your information within 10 business days.

TO SUBMIT YOUR SELF-CERTIFICATION
You must go to a DMV customer service center to submit your self-certification with a completed Driver's License Application
(DL 2P) and pay the appropriate license issuance fee if:
(A) Your current self-certification status is INTERSTATE, you are changing it to INTRASTATE and you currently hold a
CDL without an INTRASTATE (K) restriction,
OR
(B) Your current self-certification status is INTRASTATE, you are changing it to INTERSTATE and you currently hold a
CDL with an INTRASTATE (K) restriction,
OR
(C) You checked NON-EXCEPTED Interstate/Intrastate Driver, you have been granted a federal medical variance/
waiver/Skill Performance Evaluation Certificate (SPE) and your current license does NOT display a "V" restriction,
OR
(D) You checked EXCEPTED Interstate/Intrastate Driver and your current license displays a "V" restriction.
If A, B, C, or D does not apply to you, complete the Commercial Driver Information below, sign and date the Certification and
submit completed form with any necessary documentation according to the instructions shown at the top of this form.
