DONATION REQUEST FORM

We do try to help as many organizations as possible, but do to the large volume of

requests we receive not all requests can be granted. Please submit your request at

least 2 weeks prior to your event.

DONATION GUIDELINES:

•

We donate to non-profit organizations that serve our community.

•

We can only consider donation requests that are submitted on this form. We cannot

handle requests over the counter or by phone.

•

Donation requests can be made for a single rental item or a gift certificate to be used for

a raffle or door prize.

•

If a donation is made, you must make arrangements for delivery as we are unable to

deliver donated items.
