Signatures are required from homeowners from the start of the street closure to the
end of the street closure. All names and addresses must be included.
We, the undersigned, constitute at least 75% of the homeowners whose homes border the
block cited above. Do hereby agree to have the street blocked off for a block party to be
held on the date above. We further understand that once the barricades are in place there
will be NO vehicle traffic, except for emergency vehicles.
