CONFIRMATION OF ATTENDANCE
December 19, 2014
The Office of Student Financial Aid (OSFA) has been informed that all grades submitted on your behalf were not passing (W, I,
& F Grades). As a result, a calculation must be completed to determine your eligibility for financial aid. To do so, you must
submit this Confirmation of Attendance Form, signed by ALL professors for the Fall 2014 term.
If your form is not received by January 5, 2015, ALL o f your federal and state funding may be returned.
If all professors confirm attendance but no specific date is given, the calculation will be based on the midpoint
of the semester.
If you were paid financial aid based on full time enrollment and you are not able to confirm attendance in all courses,
your aid will be adjusted to the amount of credits confirmed and the calculation will be based on the last date of
attendance for the confirmed courses.
Once all funds are returned, they cannot be reinstated after the deadline.

participation or attendance, even if you have withdrawn from the class. If you are unable to get your professor’s
signature, you may have an email sent directly from the professor to finaid-wd@uncc.edu, or have the department chair
