Complete and submit this form to the Financial Aid & Scholarships Office if you will be enrolled less

than full time.

Financial aid awards are initially made based on full-time enrollment*. If you are enrolled less than full time,

funds will not pay to your student account until appropriate adjustments are made. Additionally, your cost of

attendance for both fall and spring semesters will be adjusted based on your enrollment status for the fall

semester. This may result in a reduction of your financial aid awards. You may request an adjustment to your

spring award after the spring semester census date. NOTE: There will be a delay in processing your request if

your actual enrollment is different than indicated below.

*Full-time enrollment is defined as:

Undergraduate (12 or more hours)

•

Graduate (5 or more hours)

•

Professional (10 or more hours)

Note: Wait-listed and audited courses do not count towards enrollment. Therefore, you should not submit this

form until your schedule is finalized and you are officially enrolled in all waitlisted courses.
