This form should be used only for application to an

undergraduate degree program or a certificate program at University College. Type or print clearly and answer all applicable questions.

All applicants must submit the following materials:

Completed application form.

Official transcripts from all previous college work (if applicable).

You must request that official transcripts from all colleges and universities attended be sent directly by the registrar of each institution.

Students who have attended institutions outside of the United States should provide comparable certified documents. A transcript release

form can be found on the University College website:

A $35 non-refundable application fee payable to Washington University in St. Louis.

The fee is waived for all Washington University staff.

Personal statement.

In a brief essay, describe your educational goals. Indicate any other factors which you think should be taken into consideration relating

to your application for admission. Please attach this with your application. If applying for the Creative Writing Certificate, please discuss

your experiences in creative writing and your reasons for applying to the program.

Writing samples and letter of recommendation (Creative Writing Certificate only).

Applicants to the Creative Writing Certificate program must submit one letter of recommendation.

Please also submit writing samples of either poetry (3 poems), fiction (1 or 2 stories, depending on length) or prose non-fiction (1 or 2

pieces, depending on length.
