Congratulations again on being awarded a grant from the HEI Charitable Foundation (the “Foundation”). When we

award a grant, we are entering into a partnership with you that we hope will enable your organization to reach your

goals. Grant outcomes reports are one of the ways we measure the achievements of the projects we support.

These reports also help strengthen the Foundation’s grant-making program by highlighting new directions and

issues to address.

We ask for your honest, critical attention in completing this report. We are interested in what contributed to the

success of your project, as well as reasons that made other goals more difficult or impossible to achieve.
