2016-17 NONCUSTODIAL PARENT WAIVER PETITION
The Peabody Financial Aid Office is sensitive to particularly difficult cases of divorced or separated parents. To
petition for a waiver of the Noncustodial Profile, please complete this form and provide the additional
documentation requested. A committee will review your request and render a decision. You will be notified via
email. The submission of this form does not automatically ensure that a waiver will be granted. The committee
will only review applications with all required parts (A through E) complete.

B. Personal Account
Please attach a separate, signed statement briefly explaining the circumstances surrounding why it is
impossible for your noncustodial parent to complete the CSS Noncustodial Parent Profile. Please include a
description of the following:
■ Frequency of contact you have had with him/her over the years
■ Level of financial support he/she has provided you over the years
Attach any documentation (if applicable) that would help us better understand the circumstances. Please
provide sufficient detail while limiting your statement to one page. *Please note: statements must be written
by the student applicant. A separate statement from the custodial parent may be provided at their
discretion.
C. Third Party Documentation
Please provide a separate letter from a third party who can attest to the nature of your relationship with your
noncustodial parent. A third party is person with whom you have a professional relationship such as a social
worker, therapist, attorney, clergy member, guidance counselor, etc. It cannot be from a neighbor, relative, or
friend, and it must:
■ Include the student’s full name
■ Describe the third party’s first-hand knowledge of the relationship
■ Be written on the third party’s professional letterhead
■ Be sent to our office directly from the third party
