NOTE: Pre-approvals are valid for 90 days. Loan applications received more than 90 days from the pre-approval date

will need to be re-qualified.

I authorize the lender of my loan, or its agent, to investigate my credit record to determine my eligibility for a Federal PLUS loan and

report information concerning my credit status to UEI. I authorize the release of information pertinent to this request for credit pre-

approval: (i) by and amongst the schools, lenders, guarantors, credit bureaus, the Department of Education or their agents.

I understand that my request for credit approval does not guarantee eligibility for a Federal PLUS loan nor does it qualify as a

complete application for a PLUS loan. A completed PLUS loan application and Master Promissory Note must be submitted within 60

days of the date of credit approval.

I understand that the Financial Planning Office at UEI will determine whether or not I am eligible for borrow a Parent PLUS Loan on

behalf my child. The Financial Planning Office will also determine the amount of aid my child is eligible to receive based on federal

regulations governing student loans. The actual loan amount will not exceed the cost of attendance less financial aid received.
