respondent and served on the Family Court Commissioner
and the other parties named in the original Summons for
divorce/legal separation.

Comments:
This form can be used regardless of whether there are or
are not minor children of the marriage. If there are minor
children of the marriage, in order to comply with the
Uniform Child Custody Jurisdiction Act, the form must be
notarized. If there are no minor children of the parties, the
document does not have to be notarized.

State Court’s Office.
