Evaluation of the other members of your team is important. Please fill out the bottom of this form to help

us evaluate your team’s teamwork. Do NOT share or discuss your evaluations with other team members.

Each member of the team has 100 points to allocate among all team members. Allocation should be

based upon your opinion regarding the relative contribution of each team member --including yourself --

to the overall success of the project. Some possible evaluation dimensions are listed below.

On time (arrives on time; is present at all planned activities)
