The Office of the Inspector of Independent Schools has provided this Parent/Guardian Confirmation Form to funded
independent schools to ensure consistency and transparency in the communications between the school
administration and the parent(s)/legal guardian(s) of funded and unfunded special needs students.
This form must be signed annually and attached to the student’s IEP.
As legal guardian, I confirm that _________________________________ (school) has
offered me the opportunity to review and develop my child’s Individual Education Plan
(IEP) as required in the Education Standards Order (Section 5.12) for the ________
school year.
I understand I will be given reasonable opportunity to comment on and provide input
into my child’s IEP.
I also acknowledge that the school may report my child to the Ministry of Education for
supplementary special needs funding for which my child may qualify in order to support
my child in achieving the goals and outcomes as laid out in his/her IEP.
I understand that the school administrators and staff are fully responsible for the
allocation and expenditure of special needs funding.
