is not under constraint or legal disability; that service of the Petition for Adult Adoption and Notice and Summons
has been made; and that the Adult has filed the Answer/Consent for Adult Adoption and consents to the Adoption.
IT IS THEREFORE ORDERED, ADJUDGED AND DECREED, that the Petition for Adult Adoption is hereby
granted;
and
that
the
name
of
the
adult
is
retained
or
changed
to
___________________________________, and that said adult now becomes the heir at law of the Petitioner(s);
and said adult is entitled to inherit from the Petitioner(s) any property in all respects as if such adopted person had
been the Petitioner’s child born in lawful wedlock.
IT IS FURTHER ORDERED, ADJUDGED AND DECREED, that no new Certificate of Birth based on the Decree
shall be prepared by any State Registrar.
