Directions: Ask your preceptor to assign you a patient. Find this patient’s chart in the hospital and answer the following
questions.
Identify the Medication Administration Record (MAR) in the chart. What information does this document contain?
Look at the physician’s orders section, what are the most recent medications changes?
What is the “Progress Note” section and what type of information does this section contain?
Where do you find the patient’s height, weight, and most recent vital signs and glucose readings?
Can the most recent lab value be found in the chart? If so, in what section are they located?
Using the information in the chart, calculate the patient’s creatinine clearance using the appropriate formulas and document
which formula the hospital mostly uses.
