I, Ed McCarty of “Ed’s dj Service” agree to provide DJ Services:

The price includes Dinner Music, any activities during dinner and then four hours of services

starting with your first dance. For each additional hour of DJ Services the cost will be $100.00 per

hour with a maximum of two hours.

As the undersigned, I may be required to pay, prior to final booking of your event a $200.00

non-refundable deposit payable to “Ed’s dj Service”. Further agree to pay reminder of the event cost

on or before the event date. Additionally, a meeting may be required to meet with Ed McCarty prior to

the event, fill out a DJ Service Information sheet, provide directions to and access for setup at event

location, and room layout.

Finally, in the event any of “Ed’s dj Service” equipment is damaged or items stolen by your event

guest(s), assistants must be provided to Ed McCarty to help locate the guilty parties and to ensure

reimbursement is made to “Ed’s dj Service”.
