CAHIIM Program Evaluation Plan Template
Instructions – Graduate Programs
A Program Evaluation Plan is required for all CAHIIM accredited programs to demonstrate a continuous quality improvement process. It is
designed to be used as an ongoing assessment tool with documented goals and realistic, customized and measureable program target outcomes
(at least one target outcome for each goal). For new programs applying for candidacy the following categories in the template must be completed
for each goal: goal statement, target outcome(s), steps to achieve the outcome, person responsible and time frame. Results and analysis/action
plans would be completed if actual results have been obtained in a program that has been operational for one year or more. The template model
may be modified to effectively accommodate the goal-planning process for an academic program provided that the required categories
of information are maintained.
Program evaluation goals may be related to the mission or goals to be achieved or to which the program aspires. Goals must have annual
target outcome(s) to be achieved while the goal itself may remain as a continuous program goal.
Target Outcomes must provide a metric or other measureable outcome such that the program can judge whether it has successfully been
achieved during the annual program goal review.
The ‘Steps to Achieve each Target Outcome’, the ‘Person Responsible’, and the ‘Time Frame’ must be completed for each Target
Outcome. Note that ‘Results’ and ‘Analysis and Action Plan’ are only applicable where results have already occurred or data are available.
