This Travel Reservation Form is a binding agreement between Savuti Safari & Co Inc. (Safari & Company) and any person(s) herein referred to as “Passenger”,

intending to travel with Safari & Compnay or designated tour operator. This Reservation Form constitutes an official Passenger registration for travel, and

acceptance by same of the accompanying General Terms & Conditions. All information collected will be used or disclosed only for the purposes of planning your

travel by Safari & Company and its designated tour operators, travel insuranec company and airlines.

Please print, sign and date this reservation form. Please make additional copies of this Reservation Form if there are more than 2 persons travelling. A parental

signature is required for any travelling Passenger under the age of 18 years.

IMPORTANT: All passengers must complete this form, and return by mail or fax to Safari & Company.
