Now on this ______ day of ________________________, 20_____, the undersigned judge, pursuant

to an Application filed in this matter, conducted a hearing concerning said application.

The Court finds that the application was duly and regularly filed; that the required fees have been paid

to the Court Clerk; and that five (5) days notice of said hearing was posted in the Courthouse by the

Court Clerk; that copies of the notices were mailed by the Court Clerk to the District Attorney, Sheriff

and the Chief of Police of the County Seat; and that no protests have been lodged and that it appears

the applicant is qualified.

It is therefore ordered, adjusted and decreed that a license to serve process issued by the District

Court of _________________ County, in civil cases for service within the State of Oklahoma shall be

issued by the Court Clerk upon the filing by the applicant of the required statutory bond.
