who being severally duly sworn on their respective oaths, depose and say that they
witnessed the execution of the Will of _________________________________________

that said Testator(trix) in their presence, subscribed said will at the end thereof and at the
time of making such subscription declared the instrument so subscribed to be said
Testator(trix)’s Last Will and Testament; that they, at the request of said Testator(trix)
and in said Testator(trix)’s presence and in the sight and presence of each other thereupon
witnessed the execution of said Will by said Testator(trix) by subscribing their names as
witnesses thereto; that said Testator(trix) at the time and execution of said Will, appeared
to them of full age and of sound mind and memory and was in all respects competent to
make a will and was not under any restraint.
The aforesaid subscribing witnesses further state that this Affidavit was executed
at the request of _____________________, and that at the time of the execution of this
Affidavit, the original Will, above described, was exhibited to them and they identified it
