The Petitioner(s) appearing in person, and the Court having heard the testimony and evidence
offered in support of the Petition, and being fully advised finds:
1. That the child was born on _______________ (date), at __________________________ (place of birth).
2. That the written consent for adoption by the person, persons, or agency having authority to execute the
same appears to be genuine and the child is available for adoption.
3. That the Petitioner(s) appear(s) to be of good moral character, to have the ability to support and educate
said child, and to have a suitable home.
4. That the fingerprint-based criminal history record checks of the prospective adoptive parent(s), as
reported to the Court by the county department of social services, the designated qualified individual, the
child placement agency, or Petitioner(s)
does not reveal a felony or misdemeanor conviction listed
under §19-5-207(2.5)(a)(IV), C.R.S. at least 10 years prior to the filing of the Petition for Adoption or
does reveal a felony or misdemeanor offense however the prospective parent has had no further
arrests or convictions subsequent to the conviction, and there has not been a pattern of misdemeanors.
5. The TRAILS background check completed by the Department of Human Services does not reflect a
confirmed report of child abuse or neglect.
6. The mental and physical condition of the child appears to make him/her a proper subject for adoption by
the Petitioner(s).
7. That the best interests of said child will be served by said adoption; and that the best interests and
welfare of the said child will be promoted by the issuance of this Final Decree of Adoption.
It is therefore ordered, adjudged and decreed
that the Final Decree of Adoption for the child is hereby
granted,
and
that
the
name
of
the
child
is
hereby
retained
or
changed
to
_____________________________ and that the child shall be and is hereby, entitled to all rights and privileges
and subject to all obligations of a child pursuant to statute.
