PETITION FOR UNDERAGE MARRIAGE

The Petitioner,

, hereby requests an order allowing

him/her to enter into marriage while underage, and as grounds therefor, states as follows:

The Petitioner is sixteen (16) or seventeen (17) years old and:

Both parents or guardian, or if the parents are not living together, the parent or legal custodian

with whom the Petitioner is living with, consent(s) to the marriage, as evidenced by the

signature(s) below;

Has no parent or guardian;

The Petitioner’s parent or guardian is not capable of consenting to the marriage;

The Petitioner’s parent or guardian has not consented to the marriage.

The Petitioner is under the age of sixteen (16) and:

Has the consent of each parent capable of giving consent, OR

Has the consent of his or her guardian, OR

The Petitioner’s parents are not living together and the Petitioner has the consent of his legal

custodian or the parent with whom he/she is living with.

WHEREFORE, for the reasons set forth above, the Petitioner respectfully requests that the Court order the

County Clerk and Recorder to issue a marriage license and marriage certificate form.
