Section A – Business Credit

For S Corps and/or other Corps – 2 years certified copies of IRS form 1120S including Schedule K.
For LLC or LLPs – 2 years certified copies of IRS form 1120S including schedule C.
For Partnerships – 2 years certified copies of IRS Form 1065 including Schedule K1.
For Non-profits – 2 years certified copies of IRS for 990.
Financial statements are considered audited if they have been certified by a public accounting firm and an opinion has
been recorded indicating the statements are in conformity with GAAP. Other forms are considered certified if they have
been submitted and accepted by the IRS and are not pending claims or audits concerning the documents that have been
submitted. Please include a copy of the IRS form reflecting the preparer’s signature.
